This was designed to switch on at 95% approval – when 950 out of the previous 1000 blocks were mined by software that supported BIP66. The developers had pushed out a small protocol change, BIP66 (Bitcoin Improvement Proposal 66), intended to make signature validation rules stricter. Many supposedly-confirmed transactions were being orphaned and thrown away. Different mining pools were building on different blockchains, and some were adding invalid blocks. In early July 2015, in the midst of a transaction spam denial-of-service attack, the blockchain forked. Is this technically not-wrong?
